Financial Responsibility: The total cost for Kindergarten Readiness Camp is $225. By checking the box, I understand that payment is due at time of registration. In signing up my child for Kindergarten Readiness Camp, I understand that the camp cost is non-refundable. Required

Terms of Agreement: By checking the box, I give my consent to First Priority Occupational Therapy, LLC to furnish the services listed above to my student at the location listed. I understand that there needs to be a minimum number of students (five) enrolled for the Kindergarten Readiness Camp to take place. If the minimum number of students is not met, the summer camp will unfortunately not take place. I will be notified of these changes within a week of camp taking place. Required
